Perioperative risk assessment, prevention, and treatment of acute kidney injury.

چکیده
In the perioperative period, physicians strive to provide optimal care for their patients by first performing a thorough evaluation of all organ systems. This is most important for elective and nonelective highrisk procedures and for procedures performed on critically ill or medically complex patients. The perioperative period is unique as it is a distinct event in a patient’s health where certain hemodynamic derangements and insults are somewhat predictable based on the preoperative state of the patient and the type of surgery required. It is a time when preventive therapy can be instituted for certain patient populations if their risk is adequately determined. The field of perioperative medicine is advancing in many areas. For instance, many studies have focused on the risk and prevention of perioperative cardiac events. The risk and prevention of acute kidney injury (AKI) is less well understood and shall be the topic of this chapter.

Role of acetaminophen in reducing risk of kidney injury from Rhabdomyolysis: A Narrative Review
Background: Rhabdomyolysis is a clinical syndrome that results in releasing myoglobin content from damaged muscle cells into circulation and often causes acute kidney injury (1). Different mechanisms are considered to be responsible in rhabdomyolysis and renal failure. However, free radical generation is the most important mechanism resulting in kidney injury. Thus, the effect ...
متن کاملRisk Factors of Acute Kidney Injury in Patients with Sepsis; A Cross-Sectional Study
Background and Objective: Due to the importance of acute kidney injury associated in patients with sepsis and the impact of various factors on mortality and hospital stay of these patients, this study was conducted to investigate the clinical features and risk factors for acute kidney injury in patients with sepsis. In addition, we examined whether the severity of acute kidney injury affected c...
متن کاملRole of statins in preventing cardiac surgery-associated acute kidney injury: an updated meta-analysis of randomized controlled trials
Background The prevention of cardiac surgery-associated acute kidney injury (CSA-AKI) by statins remains controversial. Therefore, the present meta-analysis including randomized controlled trials (RCTs) was performed to assess the effect of perioperative statin on CSA-AKI. Methods Two reviewers independently searched for RCTs about perioperative statin for prevention of CSA-AKI. The primary e...
متن کاملIncreased Risk of Kidney Disorders in Patients with Coronavirus 2019: A Letter to the Editor
Background and Objectives: Coronavirus 2019 (Covid-19) has become a global disease that can affect various organs of the human body. Kidney disorders are the most common disorders in patients with Covid-19. Kidney involvement manifests as proteinuria and acute kidney injury (AKI). This virus can cause acute tubular necrosis, protein leakage in the Bowman's capsule, glomerulopathy, and mitochond...
متن کاملPerioperative acute kidney injury
Acute kidney injury (AKI) is a serious complication in the perioperative period, and is consistently associated with increased rates of mortality and morbidity. Two major consensus definitions have been developed in the last decade that allow for easier comparison of trial evidence. Risk factors have been identified in both cardiac and general surgery and there is an evolving role for novel bio...
